HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

NuraRx LLC and the licensed healthcare providers who deliver care through the NuraRx platform are committed to protecting the confidentiality of your protected health information (“PHI”) as required by the Health Insurance Portability and Accountability Act (“HIPAA”) and applicable state laws.

Uses and Disclosures for Treatment, Payment, and Operations

We may use and disclose PHI without your separate authorization for:

  • Treatment — coordinating care between providers, coaches, pharmacies, laboratories, and other parties involved in your care.
  • Payment — billing, collecting fees, verifying coverage, and processing claims.
  • Health care operations — quality review, clinical training, accreditation, audits, and administration of the Services.

Other Permitted Uses and Disclosures

We may use or disclose PHI without your authorization where permitted or required by law, including:

  • Public health activities and reporting
  • Health oversight, audits, investigations, and inspections
  • Judicial and administrative proceedings
  • Law enforcement purposes consistent with applicable law
  • To avert a serious threat to health or safety
  • Research, when permitted by HIPAA and applicable law
  • Workers' compensation as authorized by law

Uses and Disclosures That Require Your Authorization

Most uses and disclosures of psychotherapy notes, marketing communications that constitute marketing under HIPAA, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except where action has already been taken in reliance on it.

Your Rights

You have the following rights regarding your PHI:

  • Right to access — request copies of your designated record set
  • Right to amend — request corrections to inaccurate or incomplete records
  • Right to an accounting of disclosures we have made for purposes other than treatment, payment, or operations
  • Right to request restrictions on uses or disclosures (we are not required to agree to every request)
  • Right to request confidential communications by alternative means or at alternative locations
  • Right to receive a paper copy of this Notice on request
  • Right to receive notice in the event of a breach of unsecured PHI

Our Responsibilities

We are required by law to maintain the privacy and security of your PHI, provide this Notice of our legal duties and privacy practices, notify you following a breach of unsecured PHI, and abide by the terms of the Notice currently in effect.

Changes to This Notice

We reserve the right to change this Notice and apply revised practices to PHI we already maintain. The current Notice will be available on our website and provided on request.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with NuraRx using the contact information below or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
